      ------
Using the information from Mr. Dearborn, in June 2002 Samuel C. Dotson initiated research in Austria, which resulted in the following additional documentation:

The birth certificate [Geburts-Schein] of Fritz Kohn, dated 12 April 1902 and stamped with the parish seal and quoting the birth records for Israelites [vol. VII is omitted in this document - sd] folio 77 of the Parish of Bennisch, appears as documentation to the order [Anordnung] of the Imperial & Royal Silesian Crownland Government [k.k. schlesischen Landesregierung] No. 1195 of 17 March 1902, which notes that the name change was decreed by the Imperial & Royal Lower Austrian Governorship [k.k. nieder-österr. Statthalterei] in Vienna on 17 December 1901: document Zl.116.103 (that is capital Z lower case L dot numerals). The Geburts-Schein itself states specifically that both Fritz' parents were of the Jewish faith.

The full volume citation ("Tom. VII, pag. 77") comes from another document (Z:6.442) from the Consistrium at Olmütz on 16 April 1902 notifying the Silesian Landesregierung that the name change has been carried out in the Bennisch records fulfilling their order of 17 March 1902.

There is another birth certificate [Geburtszeugnis] from the Bennisch parishdated 8 January 1902, apparently for document Zl.34599 for the year 1901, but the photocopies that were sent to me do not contain any document with that number, so I don't know what it is.
